HeartlandPlains Health is a not-for-profit health plan for Medicare beneficiaries with an emphasis on the physician-patient relationship, access and personal customer service. 

Founded by QualChoice Health, a subsidiary of Catholic Health Initiatives (CHI), we offer a fresh approach to Medicare Advantage where doctors, members and the health plan work together to optimize member health with costs that are affordable and predictable. HeartlandPlains Health offers a Medicare Advantage plan that emphasizes preventive care, disease management and wellness programs.
